Wanted: Installation Info for US Towers TX-472

I am ready to install a MDP version of a US Tower 72' freestanding type and All I have is the enginering calcs. I was wondering if there are any other paperwork that goes with the installation?
especially info on the type and length of the 3 anchor bolts needed .
I will build a cage but also needed info as to cage specs.
Any and all help would be greatly appriciated for this.
(I have a fax machine.)
